Subject Access Requests
Requests for personal information are known as subject access requests and can be made by completing the Subject Access Request form found here.
You are requested to
- submit your request in writing using the form (allthough you can chose not to use the form if you prefer as long as you provide the same information on the form)
- provide evidence of proof of identity
- provide us with appropriate information about what you require
Upon receipt of your request, we will respond within 1 month.
